In that scenario, the on-call surgeon is called who then has to drive to the hospital. By regulation, he has to be within a distance that can get him/her to the hospital in an alloted amount of time. If they're late, be it for traffic or car trouble, yea, that patient could suffer. Sure, the fellow or chief resident could begin the surgery in an emergency situation, but that patient could suffer.
