I think if you factor in all the deaths that have been caused by cigarette smoking--heart disease, stroke, cancer, other respiratory ailments like emphysema--and drinking--liver disease, drunk driving fatalities, etc.--the number would be greater than all the illicit drugs combined. The fact that they're readily available--legally--to the public is why the rate is so high.
