The first one to come to mind...

Sam Wyche

September 20, 1987. The Bengals led by 26-20 with six seconds remaining. Wyche called for a sweep on fourth down, James Brooks was tackled, and there were two seconds remaining. Joe Montana then tossed a 25-yard scoring pass to Jerry Rice to win the game.

I've seen Shula and at least one other coach who's anem escapes me have the QB take the ball, run all the way to the end zone and either take a knee or let the clock run out in that situation, i.e., up by more than 2 so you don't care about the safety.

Walton's QB draw with O'Brien in the 1986 playoffs against the Browns.

O'Brien gets tackled behind the line of scrimmage and it stops the clock. Jets punt, Browns get the ball move down the field in two plays, get the field goal to send the game to OT.

if only there was a movie about it....tried to eliminate from my Memory..tks:D

Parcells did that with Neil O'Donnell against the Bengals, IIRC.

Edit: Actually, he had O'Donnell throw the ball out of the end zone.

How about in Detroit in 1997, Parcells decides Leon Johnson on a halfback option pass is a better option, and he throws a pick and we do not make the playoffs?

I still can't believe the playcall, or that the refs called that an interception. The defender was laying on his back, clearly out of bounds and bobbling the ball.

yeah, I forgot about that (the guy being OB). This was 2 yrs prior to instant replay if I am not mistaken. The Vinny Play in 1998 (I remember watching it in the Bahamas in the airport coming home) was the following year against Seattle, costing Erickson his job, right? I think that's right.
