If it is a local affiliate during a local break, they'll put the same commercial in the same break to do what is called a "make good". I was in radio for a long time and we had exactly the situations Carl talks about. We didn't double up the make goods, though. Moved them to another stopset. In short, my client Paramount Pictures gives my team a bunch of money for the year and we buy out commercial time where we see fit for upcoming movie trailers/DVD releases. While I can't give you a 100% answer, I would have to say its an error. We have a agreements with each network that mandates a certain amount of separation between airing the same commercial.
